He earned his living by raising sheep and taking care of sycamore trees. He was not a prophet, son of a prophet or priest. He was an ordinary man chosen by God for a specific purpose: to prophesy to Israel. God called him, shepherds staff in hand, and sent him forth to warn his straying people. Amos message was to an age of unprecedented post-war prosperity in the middle of the 18th century. Materialism had become the order of the day. Politically, the kingdom was secure. Samaria, its capital, was considered impregnable. Under Jeroboam II, Israel enjoyed a remarkable cultural, social, and economic revival. It was the last glorious period in the history of the kingdom of Israel. The hopes and expectations of the people were lifted to the highest heights. They looked forward with great anticipation to even greater success and declared a great Day of the Lord when God Himself would take the battlefield against their antagonists. It was also a time of fervent patriotism. Everyone seemed unanimous in the conviction that God had at long last begun to fulfill His ancient promises to the nation (Genesis 9:25-27). Such was the climate into which, without warning, was catapulted the dynamic personality of Amos. The prosperity enjoyed by the Northern Kingdom was accompanied by an almost unparalleled degree of social corruption which was caused primarily by the demoralizing influence of Canaanite Baal-worship which Jeroboam II developed at the local shrines. The effect of this degenerating influence upon Hebrew society began to be felt in the corruption of justice, in willful and luxurious living of the upper classes, and in the general decay of social unity. The wealthy exhibited no responsibility towards the poor, and instead of relieving their economic distress, the rich seemed hell-bent upon depriving their less fortunate counterparts of all their property. Within a short period of time, the nation whose strength had subsisted in the mass of its independent citizens was divided into the dissolute rich and the oppressed poor. In just a few short years the poor had been reduced further to the level of serfs, and when circumstances dictated, they were frequently sold into bondage by their masters for trivial considerations. The virtual disappearance of the middle class marked a turning point in the stability of Israels life. The impending downfall and utter collapse of the Northern Kingdom is the predominant theme of the book of Amos. The basis for his prophecy is the injustice and immorality expressed in the political, economic, and religious life of the nation. It was Amos conviction that Yahweh is a God of justice and His power over the nations of the earth is evidenced by the fact that transgression of the principles of justice and social righteousness will inevitably be followed by utter ruin and decay. Anyone in the world By Michael Russell In this article we're going to briefly discuss an offshoot of the Masonic service, one for women called the Eastern Star Funeral Service. The Eastern Star organization is an offshoot of the Masons for women. The reason for this is that a woman can't become a Mason. The reasons why, at least for this article, are unimportant. However, there is an organization for women who want to be a part of the Masonic community. That organization is the Eastern Star. The only requirement for becoming a member of the Eastern Star is that the woman has to be the descendent of a Mason. Yes, there is a catch. Your father or grandfather or husband or somebody in your family has to be a Mason himself. If this one condition is met and you then become a member of the Eastern Star you are then entitled to an Eastern Star memorial service. So how does one qualify for an Eastern Star funeral service? Well, when a member of the Eastern Star in good standing, dies, they are entitled to this service. Good standing means that their dues are paid up in full and have no Masonic charges filed against them. When an Eastern Star member dies the chapter to which they belong sends a floral arrangement to the funeral home. This arrangement is in the shape of a five pointed star and with the mystical colors of the order. Before the actual service takes place however, the members of the chapter meet at their chapter building for what they call a draping ceremony. At this ceremony their alter is draped in black and a special opening of the chapter is then performed. This is, in many cases, done just once during the year to commemorate all the members who have died that year. After the draping at the chapter the members then proceed to the funeral home. At the home there is a special Eastern Star service. It is very similar to the Masonic service in some respects but very different in others. For one thing, where the Masonic service is memorized, the Eastern Star service is read from what is called a book of ritual. This book contains the entire Eastern Star funeral service and must be read word for word. In a Masonic service one person does the reading, but in an Eastern Star service there are many readers. The readings are done by various members of the fraternity who occupy specific stations as officers. Each officer reads a part assigned to their station. Each individual part is rather short but putting them all together the actual service can run 15 to 20 minutes. After the service is over the members of the chapter pay their final respects to the deceased and process out of the funeral home room. Afterwards they will meet with the family and friends of the deceased, many who are probably Eastern Star members themselves of other chapters. The Eastern Star funeral service is one of the most beautiful services of any kind that a person could attend. Anyone in the world By Jan Engles Smith I was introduced to alternative healing in a very alternative way. I had a vision. In my vision an old Indian woman approached me. When I asked who she was, she replied: I am you, I am your mother, I am your sister, I am your grandmother, I am the Earth. She had gray hair pulled back tightly in a bun, and her skin was wrinkled and weathered by life. She was wrapped in a wool blanket and smoking a pipe. She asked if I wanted to join in the sisterhood, but warned me that my life would be forever changed. I was doubtful, but kept hearing the word "trust" in the background. I nodded my head and spoke the word yes. When we finished smoking the pipe, she spoke again. "Your guidance will be provided. Your medicine that you carry is North, Wisdom, Healing, and Love. Begin at this time to start reading and learn to understand the Ways." Looking back on this experience, I see the meaning of the vision with clarity and understand fully the intent of the visitation, but at the time I was left perplexed and wondering. I had had no exposure to any indigenous teachings. I did not know what sisterhood meant. I did not know anything about the pipe ceremony; I did not even know that it was traditional for Indians to smoke pipes. I had no idea what the word "medicine" meant except as some combination of chemical elements to take when you are sick. I was engulfed in the mystery of the vision, but confused as to its meaning. Why had I had a vision? What did it mean? What, if anything, was I supposed to do with these strange images that had invaded my consciousness? Even to start reading about the Ways presented a mystery. What were the Ways? Where would I find readings that would teach me the Ways? I was involved in psychology in a very traditional way; I knew no other way. I was active at the time in the Methodist Church. In fact, I was considering studying for a doctorate in theology and perhaps pursuing a ministry. This vision was not at all a part of my existing reality system; it left me completely baffled. However, my love for God was so great that I honored the experience. I reminded myself of the many stories in the Bible where visions had occurred with wondrous messages accompanying them. I believed that this vision had come to me for a purpose I did not yet understand; however, for me to fully honor this spiritual visit, I needed to seek greater understanding. Thus began my voyage of discovery. It would prove to alter my very existence, and it is a voyage I continue to this day. The wonder of it has proven to be in the journey itself, not in any final destination. I committed to joining the sisterhood, whatever that was. I had made a contract with the vision, and I would keep my commitment. I just had no idea what to do or where to start. I felt that I needed guidance, but there was no one in my network of friends with that kind of knowledge, and only one with whom I was comfortable even mentioning the experience. It took one year before I found anyone who could help me understand what it meant. I had moved with my family from Dallas to Portland. This move created whole-sale changes in my life: a new job, new community, new church, and new friends. I did not know at the time, but I understand now, that this move was part of a major shift that would lead to a reordering of my life. Shortly after the move, I was at the Oregon coast with my husband, who was attending a convention. He burst into our hotel room saying, "Jan, come down to the vendor displays with me and meet this person who has a booth with some very unusual books. I think you will be intrigued." I followed him down and browsed through the various books. They addressed a wide range of spiritual matters, many from nontraditional perspectives. I do not remember the specific titles of the books, but I do remember feeling an uncontrollable urge to ask the sales woman if she knew anything about visions. She did not, but offered me the name of a woman who might help. I remember the excitement-the bolt of energy-that ran through me as she handed me a business card with the woman's name and phone number written on the back. I called immediately when I returned to Portland. The woman referred to herself as a channel, and claimed that she connected with the client's guardian spirits, or angels, for guidance. She told me over the phone that she felt my own guardian spirits could help me understand my vision. I caught my breath. The whole concept of guardian spirits was completely foreign to me. I had been to psychics in the past, but I had never experienced anyone who claimed to channel. I had little idea what the word channel meant or what I had gotten myself into. Yet still I was curious and felt innately that this was the right choice. I drove to her house. From that point on everything changed. Due to this shift in my awareness and my willingness to open to new possibilities, I became exposed to the people, training, and concepts that completely remolded my life and my profession. Each shift has been a step along the divine path of my development and my understanding of how we all can heal. Since the original vision, I have had numerous additional spiritual experiences. I began to reorder my life, renew my thinking, and interlace my knowledge of psychology with elements of spirituality and actual teachings from the spiritual realm. As I integrated these experiences into my own consciousness, I became acutely aware that my experiences were not only for me but were also to be used as teachings for others. The teachings and experiences hold universal messages, or basic truths, that need to be shared. As I observe clients moving through their personal journeys from head to heart, two main themes emerge again and again-the need for self-awareness and the need for redefining self. The first is a need for awareness of their own intrinsic worth. Spirit is continually providing evidence that God not only loves, but also values each being beyond all imagination. In the process of becoming yourself, you become open to this reality. The second need is for redefining self in terms of this unconditional love. It is often difficult for the mind to grasp the meaning of a love with no conditions, but the heart is capable. Unconditional love is not based on what you do, how hard you try, whether you are successful, or even if you are a "good" person. There is nothing you could do to not be loved. In the journey from the head to the heart, one comes to understand this type of love and apply it to self. When you truly understand the unconditional love from God, you move closer to being able to love yourself unconditionally. Your complete self-acceptance replaces prior doubts, disappointments, and regrets. All of life's "should have," "would have," and "could have" feelings dissipate in a newfound awareness of one's own immense worth in the eyes of God. Through my own experiences-my own head-to-heart journey-I have found that healing and the restoration of one's wholeness can be complete. Answers to the questions of life's mysteries-what is life all about, why do people become so wounded, how do people heal, why does it take so long for some to heal, why aren't people happy, and why do so many people stay entrenched in the past instead of alive in the present moment-all can be answered. The beauty of the answers is that they bring wholeness, understanding, happiness, and new beginnings for the people who employ them. What I have learned on this divine path is that life is to be enjoyed, and being whole feels wonderful! Design- Case series. Setting- Two 200-bed general community hospitals. Patients- Twelve consecutive patients from April 1985 to November 1994 - 8 with calculi, 3 with salivary duct strictures, and 1 with calculi and strictures. Results- Successful nonoperative removal of calculi in 7 of 9 patients. All four sialodochoplasties were successful. All 10 patients with successful procedures had no recurrent symptoms. Seven patients have been symptom-free for 10 months to ten years. Communication with 3 patients has been impossible recently, although these patients were symptom-free for at least 3 years. To date we have successfully treated twenty-five of twenty-eight patients for salivary duct calculi removal and dilatation of strictures. Conclusions- These methods of nonsurgical sialolith removal and sialodochoplasty were highly successful and should be used as the initial therapies for patients with these conditions. SIALOLITHIASIS and salivary duct strictures are common pathological conditions of the salivary glands and their ducts. They produce similar symptoms of swelling, pain, and infection as a result of duct obstruction. Swelling and pain usually occur during meals, when salivary secretion is stimulated.Until recently, surgery has been the standard therapy for these conditions. This approach is invasive with important unavoidable risks and complications. Potential risk of damage to the facial nerve is high during parotid gland surgery.Recently, extracorporeal shock wave lithotripsy has been introduced as an alternative treatment of sialolithiasis. Miniature lithotriptors have been developed and show some promise. However, these units are not generally available and their success rates have been variable.The mechanical removal of sialoliths and sialodochoplasty for duct stricture have been our initial approach for these diseases. These minimally invasive procedures are highly effective and avoid the known complications of surgery and anesthesia. Combining these methods with extracorporeal shock wave lithotripsy promises to further increase success rates. Results: From April 1985 to November 1994, the mechanical removal of sialoliths was successful in 7 patients with sialolithiasis: 5 with submandibular duct calculi (See Figure 1 below) and 2 with parotid duct calculi (See Figure 2 below). Four patients underwent successful sialodochoplasties, 2 for parotid ducts and 2 for submandibular ducts. One patient had sialoliths and a stricture, so the total number of successful procedures was 11. In 1 patient, the calculus was located close to the papilla, making retrieval especially difficult because of impaction. A grasping forceps was successfully used and a wide papillotomy was unnecessary. In 2 patients, calculus retrieval failed: 1 each from the Stensen and Wharton ducts. These were technical failures caused by large impacted calculi and by strictures in the distal segment of the ducts, which made mechanical manipulation impossible. There were no sialodochoplasty failures. The long-term outcomes following the procedures were excellent. No patient returned with recurrent symptoms. Seven patients remained symptom-free after clinical follow-up from 10 months to 10 years, and 3 patients were symptom-free for 3 to 5 years and then were unavailable for follow-up. Materials and Methods: Before the examination, the details of the procedure and its benefits and complications were explained to the patient. Informed consent was obtained for the sialography and for the mechanical removal of the calculi, sialodocholoplasty, or both. As an initial diagnostic examination, sialography was performed to confirm the location of the strictures and calculi. The papillae were locally anesthetized by direct injection of 1% lidocaine hydrochloride. The papillotomy was accomplished by an incision toward the duct. No sedation or general anesthesia was given. For the removal of the calculi, a 3.5F 4-wire Dormia basket (Porges, Paliseau Cedex, Salat, France), 3F Segura basket (Microvasive Co [Boston Scientific Corp.], Watertown Mass), and 3F Coaxial Sheath Grasping Forceps (Cook Urological Co, Spencer, Ind) were used. For the sialodochoplasty, 3.8F 3-mm diameter Balloon catheters (Meditech, Watertown, Mass) were used. After the papillotomy was accomplished, the papilla and adjacent salivary ducts were dilated by 3F and 4F dilators or stiff catheters of the same size. A 0.45-mm guide wire was introduced routinely to guide the balloon catheter. If there was a stricture, balloon dilatation was performed several times until full dilatation was achieved. For the calculi, a basket was placed beyond or at the calculi site and the basket was manipulated to achieve extraction. When several calculi are present, several attempts may be required. Comment: The symptoms of sialoliths and salivary duct stricture are similar: intermittent swelling, tenderness, and pain usually brought on by eating. Infection and sialadenitis are common complications. For a definitive diagnosis, sialography is imperative, especially to diagnose the presence of several calculi or to detect all strictures. A few cases of balloon-catheter sialodochoplasty and wire-basket removal of caculi have been reported, mainly in foreign journals (ref. 1-3). Also, calculus was removed by an angioplasty balloon catheter (ref. 4). The most likely surgical management of intraglandular parotid calculi would involve parotidectomy. There does not seem to be a consensus on managing calculi located between the gland hilus and anterior to the masseter muscle. Extraoral parotid sialolithotomy for calculus extraction has been performed under sialographic and ultrasonographic guidance (ref. 5). The surgical approach to submandibular calculi is influenced by the location of the stone. Palpable stones anterior to the posterior border of the mylohyoid muscle usually are extracted using a transoral incision. When the stone is posterior to the mylohyoid muscle, removal of the entire gland is recommended (ref. 6,7). The complication rate for these procedures and associated anesthesia is not negligible (ref. 8). In our independent small series during the last 10 years, we have achieved a high success rate. Contrary to other authors' (ref. 5) experience, we did not have difficulty removing parotid calculi located more than 1.5 cm from the papilla, although removal of calculi from the Wharton duct is generally easier than from the Stensen duct. The course and small size of the Stensen duct often makes instrument manipulation difficult. In our 2 cases of failure, the calculi were larger than the ducts and impacted. These ducts had long strictures in their distal segments, which made instrument approach to the calculi and manipulation impossible. A successful removal of this type of calculus was reported with a vascular snare (ref. 9). Endoscopic laser lithotripsy is unavailable at our institution. Endoscopically controlled laser lithotripsy for removal of a stone in the Stensen duct (ref. 10) and submandibular lithiasis (ref. 11) has been reported. Our 2 cases of failure could have benefited from this method. A success rate of 36% to 53% has been reported for extracorporeal shock wave lithotripsy (ref. 12). Wehrmann et a1 (ref. 13) developed a miniaturized lithotriptor, and a significantly higher percentage of patients were free of calculi (stone-free rate, 67%) after treatment. The authors did not report whether any case in this series required supplemental mechanical retrieval of calculi. In conclusion, mechanical removal of calculi and sialodochoplasty by balloon catheter are excellent alternatives to surgery. These procedures are more cost-effective, with reduced risk of morbidity when compared with the surgical alternatives. The long-term outcome following the procedure is excellent. If the mechanical retrieval of calculi fails, laser lithotripsy, extracorporal lithotripsy, or both will improve the success rate. Dr. Robert B. H.
